After the hospital team agrees that someone is ready for discharge, Muzaana can assess the personal care and practical social-care support it may provide at home.

What hospital-to-home care means

The hospital team decides when someone is ready to leave and remains responsible for the discharge decision. Muzaana does not decide or arrange the discharge date.

Muzaana assesses the social-care support it can provide once the person returns home. The agreed plan may cover personal care, meals and ordinary household routines, shaped around the discharge information and the person’s home situation.

What support can look like

What care after hospital can include

Muzaana agrees the personal and practical support after assessment. The plan reflects the person’s wishes, discharge information and what can be provided in the home.

  • Washing and dressing

    A carer can help with personal routines while encouraging the person to do the parts they want and are able to do.

  • Meals and hydration

    Support may include preparing simple meals and drinks or helping with an agreed mealtime routine.

  • Medication support

    Any support with medication is assessed, agreed and recorded in the care plan.

  • Mobility and transfers

    After assessment, carers follow the method and use any equipment agreed in the care plan.

  • Light household tasks

    A carer may wash up, change bed linen or keep the immediate living space tidy when these tasks form part of the plan.

  • Companionship and routine

    Visits can leave time for company and practical help as the person settles back into ordinary life at home.

  • Follow-up appointments

    A carer may accompany the person when the appointment, transport and practical arrangements have been agreed beforehand.

  • Recording and reporting concerns

    Carers record the support given and report relevant concerns through the named route in the care plan.

Contact Muzaana while discharge is being discussed

You can contact Muzaana when the hospital mentions a possible discharge date. This leaves time to discuss an assessment, but it does not confirm a start date or change the hospital team’s discharge decision.

Muzaana asks for the person’s consent before requesting or sharing relevant information, unless another lawful basis applies.

Prepare the information needed at home

The hospital and community teams remain responsible for discharge arrangements, health treatment and rehabilitation. Muzaana uses the relevant information made available to plan the social-care tasks it has assessed and agreed.

  • Ask the hospital team for the discharge summary and medicine instructions; any support with medicine is assessed, agreed and recorded in the care plan
  • Check which equipment the hospital or community team says is needed and who will arrange it; any support Muzaana gives with its use follows an assessment and the agreed method
  • Make sure the agreed method for moving and transfers is available after Muzaana’s assessment
  • Plan access to the home, food, drinks and essential household items
  • Keep follow-up appointment details and named professional contacts available

Social care is different from clinical recovery work

Doctors, nurses, therapists, pharmacists and community teams handle medical treatment and rehabilitation. Muzaana does not provide clinical care or rehabilitation, alter treatment or arrange discharge.

Carers provide the assessed and agreed social-care support, record relevant observations and report concerns through the named route. A health professional decides what clinical response is needed.

How care begins

Starting care is simpler than you may think

  1. Step 1: Tell us what support is needed

    Tell us what is happening, what support may help and which routines or preferences matter to the person.

  2. Step 2: Have a care conversation and assessment

    We discuss the person’s needs, choices and day-to-day circumstances, including any risks or practical points that need a clear plan.

  3. Step 3: Agree the care plan and practical arrangements

    The written plan records the agreed support, how the person wants it provided and the practical arrangements that must be in place.

  4. Step 4: Start care and review it as needs change

    Care starts after the plan and arrangements have been agreed. Reviews allow the plan to change when the person’s needs, wishes or circumstances change.

Answers before you decide anything

Who decides when someone is ready to leave hospital?
The hospital team decides discharge readiness and the discharge date. Muzaana does not decide or arrange discharge.
Can Muzaana provide rehabilitation at home?
No. Doctors, nurses, therapists, pharmacists and community teams handle medical treatment and rehabilitation. Muzaana provides only the social-care support assessed and agreed in the care plan.
Can I arrange care before the discharge date is known?
Contact Muzaana when a possible date is discussed so an assessment can be considered. An enquiry does not confirm a start, and care cannot begin until the arrangement is agreed.
Can a carer help with medication after hospital?
Any support with medication is assessed, agreed and recorded in the care plan.
What information can Muzaana share with the hospital or family?
