Discuss a homecare referral with Muzaana

If you support someone who may need care at home, call or email Muzaana to outline the referral. The first conversation can cover the person’s broad needs, their involvement and the practical next steps without assuming that care is available or agreed.

An enquiry starts a conversation. It does not confirm availability, coverage or a care start date.

Before you contact us

What helps at the first contact

A short outline lets the team understand why you are getting in touch. Consent, lawful information sharing and the person’s own wishes remain part of the discussion.

  • Your name, role, organisation and preferred contact details
  • Whether the person knows about the referral and can take part in the discussion
  • The broad type of home support being considered
  • The town or postcode where the person lives

What happens next

How a care enquiry moves forward

  1. Step 1

Tell us what support is needed

Tell us what is happening, what support may help and which routines or preferences matter to the person.

  2. Step 2

Have a care conversation and assessment

We discuss the person’s needs, choices and day-to-day circumstances, including any risks or practical points that need a clear plan.

  3. Step 3

Agree the care plan and practical arrangements

The written plan records the agreed support, how the person wants it provided and the practical arrangements that must be in place.

  4. Step 4

Start care and review it as needs change

Care starts after the plan and arrangements have been agreed. Reviews allow the plan to change when the person’s needs, wishes or circumstances change.
